Instructions - Please save and verify information in this form before you send and/or upload to IDOC.
Students are required to notify Financial Aid and Educational Financing of outside funding that they will receive. You must
complete this form even if you are not receiving outside scholarships or external payments. Simply indicate that you are
not receiving outside funding on the form before submitting. If your award receipt status changes, please submit a new
form to the Financial Aid Office once you have been notified of your updated award status.
Completed forms can be uploaded to IDOC, emailed to ugrad-finaid@columbia.edu or faxed to 212-854-5353.
