BOOK VOUCHER REQUEST FORM
This form must be turned into Student Financial Services by the first Wednesday of the first week of class to receive voucher funds.
The first disbursement will go out on Monday, June 29
th
2020 and will then be released daily throughout the first week of class.

Book voucher request for Summer 2 2020. The student understands that:
Student must be enrolled for Summer 2 2020
Student must be Pell eligible and must have turned in required paperwork to Student Financial Services (SFS).
Student will receive a book voucher for no more than $400.00 of my Pell grant award for any term
If the student’s Pell grant credit balance is below $400.00, they will receive the available balance.
Loan funds are not eligible for book voucher advances.
If student is approved for a book voucher, the student authorizes Frank Phillips College to advance a partial Pell grant disbursement to the student
prior to the first day of the class term. The amount will be posted to the student’s portal billing ledger and financial aid ledger.
Student is responsible for full repayment to Frank Phillips College of the amount advanced as a book voucher if the student does not attend the
Summer 2 2020 term.
