Billing Request Form
Florida Prepaid College Plans will be billed for tuition, fees, and costs for on-campus housing based on
the student's hours of enrollment after the drop/add period. To request changes to this procedure submit
this completed and signed form to the Office of the University Bursar as follows: (1) by scan and email to
bursar@nova.edu, (2) by fax to (954) 262-2473, or (3) by U.S. Postal Service to the address above.

I request that NSU limit the amount to be billed to my Florida Prepaid College Plan each semester to

Unrestricted Plan (may qualify for funds exceeding hours of enrollment)

I request that NSU cease billing my Florida Prepaid Plan.
I understand and agree that the NSU Office of the University Bursar will bill my Florida Prepaid Plan as
instructed on this form until (1) my funds are exhausted or (2) I submit a new Florida Prepaid College Plan
Billing Request Form prior to the drop/add deadline for each semester to request changes.
