Changes and Additions to Beneficiary, Successor Designations and Contingent
This form can be used to designate, change or name a contingent beneficiary to your registered plan (excluding RESP`s) where the law permits.
Please select your plan type and note that this designation does not extend itself to any other plan you may have.
A separate form must be used for every account under which you wish to designate, change or name a beneficiary, contingent beneficiary
or successor annuitant.

3. PRIMARY BENEFICIARY DESIGNATION Not applicable in Quebec.
Subject to applicable law, I designate the person(s) named below to receive any benefit payable under my IA Clarington registered plan on my death
if that person is living on the date of my death; if my designated beneficiary predeceases me and no other beneficiary or contingent beneficiary
has been appointed, settlement will be made to my estate. Where permitted by applicable law, I hereby revoke any previous designations. Your
spouse may automatically be entitled to benefits under a LIRA, LIF, PRIF, LRIF or RLIF.
Please refer to your Locked in Addendum/governing pension legislation and your Declaration of Trust for details on beneficiary designations as
restrictions may apply.
Note: If a beneficiary is a minor please ensure to designate a trustee for the period of minority.
The following table includes a section for contingent beneficiaries for the primary beneficiary only. If you elect more than one beneficiary
you cannot assign a contingent beneficiary.

In the event you require more room than what is provided, please reprint page one and enter the respective beneficiary number in succession i.e. page two
will consist of beneficiaries 5 – 8 page three 9 – 12 and so on.
* If the percentage allocation per beneficiary is not specifically outlined or the percentages do not equal 100%, the proceeds will be divided equally amongst
the surviving beneficiaries.

4. SUCCESSOR ANNUITANT Applicable for RIFs, LIFs, PRIFs, LRIFs and RLIFs only; not applicable for Quebec residents.
Election of spouse as successor annuitant) I hereby elect that my spouse, named below, become the annuitant under my IA Clarington RIF/LIF/PRIF/
LRIF/RLIF in the event of my death, if he or she survives me. If my spouse becomes the annuitant, my IA Clarington RIF/LIF/PRIF/LRIF/RLIF shall remain
in force and no death benefit shall be payable until the death of my spouse. You cannot assign a contingent successor annuitant.

CAUTION: If more than one beneficiary has been designated in section 3 of this document, you cannot assign a contingent beneficiary.
If you are domiciled in a jurisdiction which by law permits you to validly designate a beneficiary other than by Will, you may designate a beneficiary
to receive the proceeds of your Plan in the event of your death before the maturity of your Plan. You may make, change or revoke your designation by
written notice signed by you in a form acceptable to, and received by, IA Clarington prior to your death. Any designation, amended designations or
revoked designations will be valid on the day following its receipt by the Trustee.
Contingent beneficiary(ies) are restricted to a single primary designation only and cannot be made where multiple primary designations exist. You can
however designate multiple contingent beneficiaries on a single beneficiary.
In the event of the death of one of the beneficiaries, the attributed share will be divided equally between the remaining beneficiaries.

6. AUTHORIZATION

• The designation made on this form applies only to the account specified in section 2 of this document.
• The latest dated instrument in writing will take precedence in identifying the rightful beneficiary of a plan.
