commercial vehicles used exclusively for transporting forest products in their natural state, including logs, debarked logs, untreated ties, stave bolts,
plywood bolts, pulpwood billets, wood chips, stumps, sawdust, moss, bark, wood shavings, and property used in production of those products.

The Texas Department of Motor Vehicles maintains the information collected through this form. With few exceptions, you are entitled upon request to be informed about the information that we collect about you. Under
§§552.021, 552.023 and 559.004 of the Texas Government Code, you are entitled to receive and review the information, and to have us correct erroneous information.

Each new application or supplemental application for Forestry Vehicle License Plates must include evidence of financial responsibility or liability insurance issued
by an insurance company or surety company authorized to write Motor Vehicle Liability Insurance in Texas.
> The registration fee for Forestry Vehicle License Plates is the same as the regular vehicle registration fee prescribed by law (based on the gross vehicle
weight).
> Due to Federal law, proof of payment of the Federal Highway Vehicle Use Tax (HVUT) is required when licensing vehicles 55,000 pounds or more gross
weight or combined gross weight. Proof is not required on new or used vehicles being titled and registered within 60 days of the date shown on the bill of
sale. Acceptable proof of payment is a receipted copy of an Internal Revenue (IRS) Schedule 1 (Form 2290) listing the equipment by vehicle identification
number, up to 21 units, or a copy of Form 2290 and a copy (front and back) of the cancelled check.
> The registration year for Forestry Vehicle License Plates is April 1, through March 31, of the succeeding year. The registration fee will be prorated monthly if
the plates are issued for less than 12 months.
> New applications for Forestry Vehicle License Plates are processed by one of our Regional Service Centers located throughout the state. Texas title must be
in the applicant's name before application for a Forestry Vehicle License Plate can be approved. If an application for title in the applicant's name has been
filed, attach a copy of the Title Application Receipt. If the vehicle is leased, attach a copy of the lease agreement.
> If an applicant wishes to obtain Forestry Vehicle License Plates for additional vehicles, a supplemental application should be filed with the Regional Service
Center that handles the account. Forestry Vehicle License Plates should be renewed annually through the Commercial Fleet Services section. Renewal forms
will be mailed to the applicant each year prior to the plate's March 31, expiration dates. The mailing address for Commercial Fleet Services is:
