FA SAP Appeal
2020-21 Satisfactory Academic Progress (SAP) Appeal Form
Federal regulations require students to maintain Satisfactory Academic Progress (SAP) to be eligible for financial aid.
The three components are 1) cumulative GPA (must be 2.0 or greater), 2) completion rate (must pass at least 67% of
attempted classes), and 3) maximum timeframe (must be within 150% of program maximum). More information about
EFSC’s SAP standards is available on the Office of Financial Aid’s website and in the EFSC Catalog.
Deadlines*: Fall 2020 11/24/20 | Spring 2021 04/22/21 | Summer 2021 07/22/21
* Deadlines listed are for the full term. If enrolling only in shorter parts of term (Minimester A, Session I, or Summer B), appeals
must be submitted no later than two weeks prior to the last day of the part of term for which the aid is sought.
Late appeals will be considered for the next semester.

I have attached a typed, signed, detailed letter explaining
1) how an extenuating circumstance beyond my control prevented me from meeting SAP requirements,
making sure to address all relevant semesters AND
2) what has changed that will allow me to be successful in upcoming semesters.
Extenuating circumstances may include, but are not limited to:
Documented medical conditions or serious illness
Death of an immediate family member
Other emergency circumstances (i.e. natural disaster, etc.)
I have attached date specific supporting documentation from a disinterested third party. Documentation must
be relevant to the terms in which academic difficulty was experienced. Acceptable documentation may include
but is not limited to:

Letter (not prescription) from a physician or counselor indicating the dates of the medical care (details
of the medical care can be redacted)
Copy of a death certificate, obituary, or third party documentation of the death of a family member
Teacher Notification from EFSC’s Student Access for Improved Learning (SAIL) Office
Accident Report / Police Reports / Court Records, etc.
Note: Do not submit original documentation. Please provide legible copies.

documentation from a disinterested third party, it is highly recommended that you submit it with your appeal.

E. Appeal Conditions
All appeal decision notifications will be sent via Titan e-mail.
All appeal approvals are contingent upon the completion of your financial aid application.
The Office of Financial Aid will not approve appeals that are incomplete or lack documentation.
If an appeal is denied, it may be resubmitted only if additional documentation not previously submitted is
provided.
If your appeal is denied, you need to be prepared to pay tuition, fees and other educational expenses until
such time that you are making Satisfactory Academic Progress and regain eligibility for financial aid.
Appeals will not be approved retroactively. You must appeal by the semester deadlines at the top of this
form.
