The California Student Aid Commission (CSAC) requires the completion of this form to verify your eligibility to receive
a Transfer Entitlement Cal Grant award. Read this form carefully, then complete ALL questions, sign, date and
mail to the address listed on the back of this form. Failure to return this form will adversely impact your ability
to be considered for the Transfer Entitlement Cal Grant. Please see other side for instructions.
Transfer Entitlement Program Requirements:
You have been given preliminary award consideration because CSAC received your California Community College GPA
of at least 2.40. In addition to the GPA, every Transfer Entitlement Cal Grant applicant must also meet the following:
attended and completed at least 24 semester units at a California Community College and then transfer and attend a qualifying California
institution that offers a bachelor’s degree for the 2018-19 year; AND
graduate from a California high school (unless military orders required the student or the student’s parent or guardian to be out-of-
state at the time of graduation), or complete a high school graduation equivalency exam (including GED, HiSET, TASC or the
California High School Proficiency Exam) on or after July 1, 2000 and be a California resident at the time; OR
if you did not graduate high school or obtain the equivalent but were a California resident at age 18.

In order to receive further consideration for a Cal Grant award, you must complete all questions, sign and return this form to
CSAC. If you are not eligible for a Cal Grant Transfer Entitlement award, you may be considered for a
Competitive Cal Grant award. DO NOT LEAVE ANY QUESTION BLANK.
Instructions for filling out the Student Certification section:
When completing this form, please print clearly using black ink only.
Answer each question by filling in the response or checking the appropriate box as it applies to your situation.
1.
Enter your first name, middle initial, last name and date of birth. Print your CSAC ID as it appears on your CSAC
notification letter/email.
2.
Print the month and year you graduated from high school or achieved the equivalent by passing the GED,
HiSET, TASC or CHSPE. For example; June 2015. If you did not graduate High School or obtain the equivalent
(GED, HiSET, TASC or CHSPE) please write ‘Did Not Graduate’ on the line provided.
3.
"High School" means a secondary school accredited by Western Association of Schools and Colleges (WASC), or
another regional accrediting association if the secondary school is not in the WASC region, or which has a
University of California approved course list.
4.
Mark this box only if you were a California resident at the time of high school graduation but gradu-ated from a
high school outside of California due to you or your parent or guardian’s military orders from the United
States Armed Forces. If you graduated from a California high school, leave this box empty.
5.
Print the name of the high school from which you graduated. If you completed high school by passing the GED,
HiSET, TASC or the CHSPE, please write in “GED,” “HiSET,” “TASC,” or “CHSPE”.
6.
If you graduated high school or completed high school by passing the GED, HiSET, TASC
or CHSPE please enter the city and state of graduation/completion.
7.
If you applied for Cal Grant by completing the CA Dream Act Application, please print the date you started living
in California. Otherwise, please print the date you became a California resident. If you have resided in
California since your birth, please put your date of birth.

If you attended a community college during the 2017-18 academic year, please list the college name on the line
provided. If you did not attend college in 2017-18, please enter “Did Not Attend”.
9.
Print the name of the four year college you expect to attend during the 2018-19 academic year. For example, "CSU
Humboldt".
